Advanced Directives - Madison Senior Center

About:  Join us for this 2-part program on Advance Directives. Join Ellen Taylor and Kayla Olson, Dementia Care Specialists at the Aging & Disability Resource Center of Dane County, in a discussion regarding how to talk about what matters to you and have a say in your health care. You will learn about advance care planning options and next steps in putting it all together. 

The second hour of the session will be drop-in office hours to ask some questions and complete the Wisconsin state forms yourself. We will provide the state forms and there will be non-ADRC on-site witnesses and a notary present from Associated Bank. 

 

What to know/bring:  Forms and pens will be supplied. This program will be 2 hours in duration, with the second portion being more of a drop-in/open forum for questions and completing forms.
