Event Description
Join us for this 2-part series. The first session, on April 7th, will provide education on what it means to Advance Care Plan and why it’s important. Join Ellen Taylor and Kayla Olson, Dementia Care Specialists at the Aging & Disability Resource Center of Dane County in a discussion regarding how to talk about what matters to you and have a say in your health care. You will learn about advance care planning options and next steps in putting it all together.
The second session, on April 21st, will provide drop-in office hours to ask some questions and complete the Wisconsin state forms yourself. We will provide the forms. There will be non-ADRC on-site witnesses and a notary from Old National Bank present at the April 21st session.
What to know/bring: Forms and pens will be supplied. This program will be 1.5 hours in duration each time, with the second being more of a drop-in/open forum for questions and completing forms.
Space is limited and registration is required.
